Bring them with you to your first appointment. If you do not have access to a printer, that’s no problem. Come about 10 minutes early and I’ll provide a set for you.

*Did you ever wonder why you are asked to fill out a ton of information for every healthcare provider you have ever seen? Well, I can’t speak for every provider, but I can tell you why I want all this information. The information you provide on these forms gives me a pretty good overall picture of your health. Health problems don’t exist in a vacuum – they are always in context of your whole life. Here are some examples of what I mean and why that could be important:

  • Got knee pain? That might have to do with a football injury you had when you were a kid or it might have to do with your heart problems (true story).
  • Got allergies or the sniffles? If your thirst has increased and you feel hotter than you used to, that means there’s a whole different reason you are having symptoms than it would if instead of feeling hotter, you are feel colder than you used to and your thirst has totally gone away.
  • Are you tired all the time? What foods you crave will tell me a lot about where that’s coming from.

This is why I ask all these questions and want all of this information from you. I want to take care of your symptoms, but I also want to get to the heart of why those symptoms showed up to get your attention in the first place.
